What is the problem?
  • Expensive Medical and Nursing staff spend 2.5 hours per day in taking and recording clinical measurements. This costs a practice of 5000 patients an estimated $70,000 dollars per year.
What is the solution?
  • Care Kiosk is a physical measurement kiosk that records height and weight (calculates BMI), blood pressure, body composition (i.e., fat/muscle percentage), temperature, heart rate and oxygen saturation.
  • Traditionally these measurements were taken by a healthcare professional. Using Care Kiosk frees up time for clinic staff and gives patients the flexibility to get measurement checks outside their scheduled appointment times.
  • The data produced is sent seamlessly to the patient record. In 2024 further updates will allow clinical rules to be applied to the results. Output from these rules, will include active rule-based direction to the patient e.g., notification to get a blood test, and clinical decision support for the clinician

Patient/Practice InBox - the problem
  • In NZ there were 46 million community and hospital laboratory reports, outpatient and hospital discharge summaries, radiology reports and letters delivered to the inbox.
  • GPs spend increasingly large lengths of time reviewing the inbox contents.
  • Often leads to doctors working overtime to ensure that results are reviewed and acted upon and significantly contributes to burnout.
  • Seeing values in isolation doesn’t allow analysis or trends and the greater clinical context.
  • Can lead to significant results or trends being missed or inappropriately labelled normal.

Care Triage enables patients to be safely and efficiently directed to the appropriate consultation type with an appropriate health care provider or service by non-clinical staff.

Currently, many patients request to see a GP unaware that they could be seen by an alternative health provider or service. Care Triage is a decision support tool for non-clinicians to use.

Care Triage is easy to use and backed by robust clinical algorithms. The application integrates with the Patient Management System (PMS) and can therefore "detect" whether the patient has pre-existing complex medical issues, or is at high risk e.g., immunocompromised. Using the data provided by the patient and the data from the PMS, the patient is then directed to the correct appointment type with the appropriate provider or service. A full summary is automatically written back to the daily record and the patient outbox.

Care Manager is a tool which allows for the management of long-term conditions in individuals or populations. It provides a common interface for a range of clinical users providing a patient-centric approach. The interoperability of Care Manager with the remaining SmartCare suite enables detailed care plans to occur and align effective and efficient use of the clinical workforce.
